Impact of a vacuum cushion on intrafraction motion during online adaptive MR-guided SBRT for pelvic and para-aortic lymph node oligometastases

Background and purposeVacuum cushion immobilization is commonly used during stereotactic body radiotherapy (SBRT) to reduce intrafraction motion. We investigated target bony anatomy motion (translations rotations) online adaptive SBRT on an MR-linac for pelvic/para-aortic lymph node metastases with without vacuum cushion.Materials methodsThirty-nine patients underwent 5x7 Gy a 1.5T MR-linac, 19 were treated cushion, 1 patient sequentially without. Intrafraction was calculated nodes (GTVs) nearby anatomy, three time intervals (pre-position verification (PV), pre-post, PV-post, relating the MRI scans) per treatment fraction.ResultsVacuum significantly reduced anterior-posterior translations pre-PV pre-post intervals, interval GTV (p < 0.05). Mean reduction in posterior direction 0.7 mm (95% confidence 0.3–1.1 mm) (mean = 32 min). Shifts other directions not reduced. More occurred than PV-post 16 min both); did beam-on period.ConclusionA reduces SBRT. This found first session.
